Health Information and Health Screening
This form is to indicate your preference in health screening options and to make current any student health conditions we need to be aware of.

Student Health Services Opt Out form (grades 6-12)
Use this form to select if you will opt out of illness assessment, injury assessment, wellness assessment, or mental health assessment.

Bus Policy Form
This is a required form for all students. It lines out the expectations for riding a bus. Students will need a signed form to ride school transportation including routes, field trips, activities, or any other activity requiring transportation.

Student Computer / Internet Acceptable Use Contract
This form is required for all students.

Use this form to submit any changes to our information (contacts, phones, email, court orders, etc). If you are requesting any changes to custody or medical, please upload the supporting documents.

Athletic / Activities Code of Conduct & Parent Agreement
This form is for any student who will play a sport or be a part of a student organization (FFA, Skills, etc)

Fill out this form to apply for reduced or free lunch and breakfast
